Rumination – why you cannot “switch off” negative thoughts and what you can do

What is rumination

Rumination is a mental process in which the mind returns repeatedly to the same thoughts, worries or scenarios without finding a solution or a constructive direction. It is like a closed circuit in which thoughts keep spinning, often centred on past mistakes, fears, negative scenarios or excessive self-criticism. Although it may seem that rumination helps to find clarity or to understand a problem, in reality it rarely leads to resolutions. It works more like a magnet for attention, capturing important cognitive resources and locking the mind into a state of analysis with no end point.

In neuroscience, rumination is associated with excessive activation of the brain's default mode network, a system involved in introspection and self-referential thinking which, when out of balance, can fuel anxiety and depression. In practice, the brain recycles the same information in an unproductive attempt to control emotions, which raises inner tension even further.

The impact of rumination on physical and mental health is consistent and well documented. At the psychological level, rumination amplifies depressive symptoms, sustains anxiety, reduces motivation and lowers the ability to make clear decisions, because it consumes a large part of the available mental resources. It can also distort the perception of reality, turning manageable difficulties into problems that seem insurmountable, which feeds a vicious circle of helplessness. On the physical side, rumination is correlated with rising levels of cortisol, the stress hormone, which over time weakens the immune system, disturbs sleep and can affect digestion, blood pressure and cardiovascular health. Persistent rumination can lead to exhaustion, irritability, muscle pain or migraines, because the body stays locked in a state of chronic alert. In essence, rumination is not merely an unproductive mental habit, but a real risk factor for emotional and physical imbalance, a process worth observing, addressing and regulating through awareness techniques, psychological interventions and appropriate support.

Brain mechanisms involved in rumination

1. Activation of the brain's default mode network (Default Mode Network - DMN)

The brain's default mode network, or DMN, is involved in introspection, self-referential thinking and reflection on the past or the future. Normally this network activates during periods of rest and self-reflection, but in rumination the DMN becomes hyperactive and persistent. This leads to repeated returns to negative thoughts, self-criticism and imagined scenarios of failure or loss, blocking the ability to switch to solution-oriented cognitive processes. In practice, the brain stays “caught” inside itself without producing concrete solutions, which amplifies anxiety and sadness and predisposes to chronic stress.

2. Trauma and adverse childhood experiences

Traumatic or abusive experiences in childhood can recalibrate the stress response systems and emotional memory, creating a predisposition to rumination. Early trauma changes the way the amygdala and the prefrontal cortex interact: the amygdala becomes hyperactive, detecting danger in an exaggerated way, while the prefrontal cortex - responsible for regulating emotions and for rational planning - can be inhibited or less efficient. This combination makes people with trauma return frequently to painful experiences, analysing them repetitively and without resolution, which maintains emotional stress and strengthens the neural connections that favour rumination.

3. Chronic stress and emotional overload

Persistent stress activates the hypothalamic-pituitary-adrenal axis, raising cortisol levels and keeping body and brain in a prolonged state of alert. This affects the functioning of the prefrontal cortex and reduces the capacity to regulate repetitive thoughts. In this state the brain constantly seeks explanations or solutions for perceived stressors, but the repeated return to the same negative scenarios amplifies rumination. Over time, chronic stress and rumination can reinforce each other, contributing to anxiety, depression and cognitive fatigue.

4. Anxiety and mood disorders

Anxiety and depressive disorders are associated with hyperactivation of the DMN and with difficulty in “switching networks” towards the dorsolateral prefrontal cortex, which is responsible for solution-oriented thinking. People with these disorders tend to focus excessively on perceived problems, often exaggerating the negative consequences and amplifying self-criticism. In these cases the brain “gets stuck” in repeated cycles of thought, and prolonged negative emotions strengthen the neural connections that favour rumination.

5. Recent experiences of loss and emotional trauma

Recent negative events, such as the loss of a loved one, divorce or job loss, activate both the DMN and the amygdala and the hippocampus, structures involved in processing emotional memory and stress. The brain tries to understand “why” the event happened and how the pain could be avoided in the future. Without adequate support or emotional regulation strategies, this process can become repetitive, keeping the person in a circle of rumination that prolongs the suffering and reduces the capacity to adapt.

Risk factors of untreated rumination

Untreated rumination can have significant consequences for mental and physical health, and recognizing the risk factors is essential for preventing the symptoms from worsening. These factors not only increase the frequency and intensity of repetitive thoughts, but can also amplify vulnerability to mood disorders, anxiety or chronic stress. People who present one or more of these factors are more likely to maintain rumination and to have difficulty with emotional regulation, which can significantly affect quality of life and social relationships.

The main risk factors include:

  • Traumatic experiences and abuse - traumas in childhood or in adult life can leave the brain stuck in repetitive cycles of thought as it tries to process unresolved emotions.
  • Mood disorders and anxiety - depression, anxiety disorders or bipolar disorder favour persistent rumination and excessive self-criticism.
  • Perfectionism and high self-criticism - people who constantly criticize themselves or hold unrealistic expectations of themselves are more prone to repetitive thinking.
  • Chronic stress and emotional overload - prolonged exposure to stress keeps the nervous system activated and encourages a constant return to perceived problems.
  • Social isolation - the lack of emotional support and of close connections can amplify repetitive thoughts and the feeling of helplessness.
  • Lack of emotional regulation skills - difficulty in managing negative emotions makes the mind look for solutions through rumination, which most of the time brings no resolution.

The limits of cognitive approaches in managing the tendency towards rumination

1. Excessive focus on thoughts can amplify rumination

Cognitive approaches, such as cognitive restructuring, place the emphasis on identifying and modifying negative thoughts. However, for people with a tendency towards rumination, focusing on thoughts can have the opposite effect: the mind stays caught in repetitive cycles of analysis. Instead of lowering the intensity of negative thoughts, this can increase excessive awareness of them and self-criticism, thereby consolidating the neural network involved in rumination. In practice, the attempt to “correct” thoughts can be perceived by the brain as a problem to be solved, which fuels repetitive thinking rather than reducing the suffering.

2. Ignoring the somatic and emotional components

Strictly cognitive approaches tend to focus on thoughts, rationality and logic, often neglecting the bodily reactions and intense emotions that sustain rumination. Stress, muscle tension, an accelerated heart rate or sensations of anxiety all contribute to maintaining the cycle of repetitive thinking. Without interventions that integrate bodily regulation, such as conscious breathing, progressive muscle relaxation or somatic techniques, simply modifying thoughts may be insufficient. Body and mind are connected: if stress signals are not reduced, the brain keeps feeding rumination even in the presence of “corrected” thoughts.

3. Limits in the face of trauma or deep emotional experiences

Traditional cognitive approaches are not always sufficient for people with trauma, abuse or significant losses. In these cases rumination is often linked to emotionally charged memories and to changes in the neural circuits involved in memory processing and emotional regulation. Trying to “think rationally” or to rewrite thoughts may not be enough to reduce amygdala activation or DMN hyperactivation. Cognitive approaches therefore need to be complemented by somatic and therapeutic methods, or by methods based on processing emotional experiences, in order to be truly effective.

How neurofeedback works and what it offers in the treatment of rumination

Neurofeedback is a technique for regulating brain activity that uses real-time feedback to help people understand and adjust their own neural patterns. In practice, electrodes placed on the scalp record brain activity, and the information is converted into visual, auditory or tactile signals that the patient perceives immediately. This feedback allows the brain to observe its own activation patterns and to learn to change them in a beneficial way. In rumination, where there is hyperactivation of the default mode network (DMN - Default Mode Network) and of the circuits associated with excessive introspection, neurofeedback can help reduce repetitive activation, allowing the brain to shift more easily towards states of focus and emotional regulation. Through repeated practice, the brain learns to build more balanced connections between the prefrontal cortex and the subcortical structures involved in emotion, which reduces the tendency to return obsessively to negative thoughts.

Instead of focusing solely on identifying and changing repetitive thoughts through cognitive exercises, neurofeedback addresses the biological basis of rumination, the way neural circuits amplify themselves in cycles of repetitive thinking. This technique can improve emotional regulation, attention and the ability to stay present, thereby reducing anxiety and the associated depressive symptoms. Moreover, neurofeedback can be used alongside psychotherapy, mindfulness or breathing exercises, increasing the effectiveness of traditional interventions and offering a practical tool for the direct control of brain activity.

In practice, neurofeedback works through repeated sessions, each one serving to strengthen healthy brain patterns and to reduce the hyperactivity of the networks involved in rumination. The results do not appear instantly, but over time the brain starts to respond more flexibly to stress and negative emotions. People who follow neurofeedback often report a decrease in the frequency of repetitive thoughts, greater mental clarity and an increased ability to interrupt negative cycles before they become overwhelming. Neurofeedback thus offers concrete and measurable support for managing rumination, turning a persistent tendency towards negative thinking into a more controllable and more balanced process, opening the way towards lasting emotional regulation.

When neurofeedback therapy for rumination is indicated and who it is for

Neurofeedback is an intervention aimed at people who experience chronic rumination or repetitive negative thoughts that interfere with everyday life. This therapy is not limited to an age group or a profession; it is useful for anyone with difficulties in emotional regulation, reduced concentration or intense stress.   • Obsessive-compulsive disorder (OCD) - repetitive worries and mental scenarios can be reduced by adjusting the patterns of neural activity.
  • Post-traumatic stress disorders (PTSD) - intrusive memories and obsessive thoughts linked to the trauma can be regulated more effectively, reducing excessive emotional reactivation.
  • Bipolar disorder and other mood disorders - neurofeedback can help stabilize emotional states and reduce the cycles of negative thinking.
  • Attention disorders (ADHD) - although rumination is not the main symptom, neurofeedback helps regulate attention and reduce the distraction caused by repetitive thoughts.
  • Emotional or physical trauma and abuse - people with adverse experiences can develop chronic rumination linked to unresolved memories and emotions; neurofeedback supports emotional regulation and the reduction of limbic hyperactivation.

The therapy is therefore aimed both at people with a clinical diagnosis and at those who face persistent rumination without a formal diagnosis, but who experience chronic stress, difficulty in regulating emotions or a significant impact on quality of life.
