What is sciatica?

Sciatica is leg pain originating in one or more nerve roots in the lumbar and sacral area (usually L4-S2).

The sciatic nerve is responsible for controlling the muscles of the back of the thigh and all the muscles located below the knee.

It carries sensory impulses from the back of the thigh, the back and side of the lower limb and the entire sole of the foot. Sciatic pain therefore runs along the length of the sciatic nerve: it starts in the lower back and radiates to the buttock and leg.

The causes of sciatic pain:

  • A herniated disc that irritates or compresses a nerve root – 95% of sciatica cases occur through damage to the L5 or S1 root caused by a lumbar disc herniation;
  • Spinal stenosis (narrowing of the canal);
  • Spondylolisthesis – the forward slipping of one vertebral body over another;
  • Osteoarthritis – the deterioration of the tissues of the lumbar spine can compress or irritate one or more roots of the sciatic nerve;
  • Injuries to the lumbar spine and sciatic nerve – fractures of the femoral head, injuries to the pelvis;
  • Tumours in the spinal canal, which compress the nerve roots;
  • Haematomas (a build-up of blood) or abscesses;
  • Piriformis syndrome – the compression of the sciatic nerve at the level of the piriformis muscle, which can cause symptoms such as burning pain and numbness extending throughout the lower limbs.

Risk factors

  • Age – age-related changes in the spine are the most common causes of sciatica;
  • Obesity – by increasing the stress on the spine, excess body weight can contribute to the spinal changes that trigger sciatica;
  • A sedentary lifestyle – people who stand for prolonged periods or have a sedentary lifestyle are more likely to develop sciatica than active people;
  • Diabetes;
  • A history of spinal injuries;
  • Osteoarthritis;
  • Smoking – the nicotine in tobacco can damage spinal tissue, weaken bones and speed up the wear of the vertebral discs.

How does sciatica manifest itself?

Lower back pain– the painful contracture of the muscles along the spine, extending down to the leg.

Radicular pain – felt as a persistent burning sensation or a sharp, stabbing pain that starts in the lower back or buttock and radiates down the front or back of the thigh and/or calf as far as the heel; it is aggravated by physical effort, abdominal contraction and the strain of coughing, and eases at rest. It is frequently accompanied by tingling and numbness, all along the same path.

Sphincter disturbances– bladder emptying, defecation, potency.
